Health Assessments

The process of a health assessment enables time to be taken out to do an in depth assessment of the health needs of the older patients, including social and physical aspects.

This should help pinpoint potential health problems for the older individual, and increase quality of life.

Who is able to have a health assessment?
People 75 years and over, and Aboriginal & Torres Strait Islander people 55 years and over. They cannot be living in an aged care facility or a nursing home.
 

MUST DO:

  • Check the person fits the eligibility criteria ensuring that they have not had a health assessment in the last twelve months by asking the patient and checking in their notes.
  • Gain consent for a health assessment to be carried out.
  • Complete a comprehensive biophysical and psychosocial assessment of the person.

 The basic assessment can be completed by the practice nurse and should include:

  • Information about their activities of daily living

  • Physical functions- activity levels, hearing, vision, potential for falls and home hazards

  • Assess BP, pulse rate and rhythm

  • Assess diet, foot care, hygiene etc

  • Medication review, and assess the level of understanding about their medications

  • Psychological function level - mood, cognition, memory e.g. mini mental health exam

  • Social function and supports    

  • Vaccinations  

A detailed written record of this process should be completed as the physical assessment occurs, and be signed by the patient, including any recommendations the GP can make to improve their health care. The patient should be offered a copy.

Aboriginal Person’s Health Check (Health Assessment) 

The purpose of this adult health check is to ensure that Aboriginal and Torres Strait Islanders people receive the optimum level of health care by encouraging early detection, diagnosis and intervention for common and treatable conditions that cause considerable morbidity and early mortality.

Who is able to have a health assessment?
A person that is of Aboriginal or Torres Strait Islander descent and aged 15 to 54 years.  

MUST DO

  • Check the person fits the eligibility criteria ensuring that they have not had a adult health check in the last 2 years by asking the patient and checking in their notes.
  • Gain consent for an adult health check to be carried out.
  • Complete a comprehensive biophysical and psychosocial assessment of the person. If there is no comprehensive medical history existing for the person one must be taken.

The basic assessment can be completed by the practice nurse and should include:

  • Must be taken BP, pulse rate and rhythm

  • Must be taken BMI oral exam, hearing and ear exam, urinalysis.

  • Collect information about the person’s health risk factors including cardiovascular.

  • Carry out investigations should include fasting blood sugar and lipids, pap smear, STI testing and mammography.

  • The GP must initiate any interventions that they believe will met the identified needs of the patient and give preventive health advice.

These activities can include:

  • Starting a treatment, making referrals or providing an immunization.

  • Providing education and advice in relation to smoking, nutrition, alcohol or other substances and physical activity

  • Providing education and advice about reproductive health issues, social and family issues.            

  • Develop a simple strategy for good health for the patient according to their needs and actions the patient should take based on the information from the health check.

  • The strategy should be developed with the patient and must be written down.

  • A detailed written record of this process should be made as the health check occurs and a copy offered to the patient, including any recommendations covered by the health check, including a simple strategy for good health for the patient.
