Prevention of Type 2 Diabetes - Lifestyle Modification Program (LMP)


Diabetes is now one of the largest contributors to the disease burden in Australia, and type 2 diabetes accounts for 92% of all diabetes cases. However, Type 2 Diabetes is also largely preventable by controlling associated lifestyle risk factors such as overweight and obesity, physical inactivity and poor diet.

The Prevention of type 2 diabetes program is designed to help delay or possibly prevent the onset of type 2 diabetes by targeting the associated risk factors.

The Dubbo Plains Division of General Practice has initiated an Accredited Lifestyle Modification Program (LMP). The Program will be trialled in Dubbo and will aim to reduce the risk of, or delay the progression of type 2 diabetes. The group program will be facilitated by a qualified Dietitian and supported by a Registered Nurse and Diabetes Educator.

Patients who are aged 40 – 49 years, are at high risk of type 2 diabetes based on the AUSDRISK tool, have  had diabetes excluded and completion of one of the following Health Assessment item (Type 2 diabetes risk review, 45-49 year old health check or Indigenous adult 15-54 year old health check) may be referred to the program.

AUSDRISK is a short questionnaire to help both health professionals and patients measure someone's risk of developing type 2 diabetes.

The LMP will typically be a series of group education and motivation sessions supporting lifestyle change. 

LMPs will provide information on:

  • the risks of diabetes and their relationship to lifestyle risk factors;
  • the importance of regular diabetes screening;
  • nutrition advice and education;
  • physical activity advice;
  • behavioural strategies to support the adoption and maintenance of healthy lifestyle changes;
  • smoking cessation and alcohol reduction advice if required; and
  • information about community resources to provide support in maintaining lifestyle

 

All referrals must be made using the General Practitioner Referral Form and should include the completed AUSDRISK Assessment Tool. All referrals to be faxed or sent via ARGUS (ensure contact details are updated) to the Division (6884 0198), attention Life style Modification Facilitator - Alisha Cooper. Alisha will contact patients regarding Program details ..

There is a $20 patient co-payment for this program.
