Rural Palliative Care Project 2008-2010

Dubbo Plains Division of General Practice is one of 36 rural and remote Divisions who have received funding from the Australian Government Department of Health and Ageing to implement the Rural Palliative Care Project.

The focus of the project is to identify:

  • Gaps and needs in Palliative Care service delivery
  • Sustainable shared care Palliative Care service delivery model
  • Annual education activities for General Practice
  • Sustainable communication processes between General Practice and Cancer and Palliative Care services.

A Local Advisory Panel (LAP) has been appointed to oversee the implementation of the Project. LAP Membership includes:                                              

  • Local and regional GPs
  • Greater Western Area Health Service Area Manager Cancer and Palliative Care Services
  • Oncology Nurse Unit Manager
  • Palliative Care Clinical Nurse Consultant
  • Cancer Nurse Co-ordinator
  • Consumer representative
  • Division Aged Care & Palliative Care Program Manager
