What is a Case Conference?
A Case Conference is where the GP designates time for
conferencing with other health providers to plan care for people with chronic
conditions and multidisciplinary care needs. It can be conducted face-to-face,
via the telephone or videoconference link.
Items numbers depend on:

The length of the conference
-
Whether the GP is organizing and coordinating a conference or
participating in another care providers conference
-
Whether the conference relates to hospital discharge
These Items give health care providers an opportunity to
determine current health status and problems that a person may be experiencing
and to consider the most effective way of supporting them.
Criteria:
-
Chronic condition > 6 months, terminal or have multiple health
problems
-
Can be a resident of an aged care facility or nursing home

Obtain the consent of the client

Identify at least 2 allied health providers that can meet the
health needs of the clients.

Coordinate between these allied health providers, a suitable
face-to-face appointment time, or if necessary a telephone or videoconference
link. (The practice nurse can do this.)

Organise the most suitable place for the case conference to
occur.

Notify the client of the arrangements and who will be
participating in the conference. (The client does not need to be present
during conference).
